This is the written consent form each authorized campaign contact completes during AmericanRed.ai client onboarding to enroll in operational text alerts. Enrollment is optional and is not a condition of service — escalation alerts can be delivered by email or phone call instead. The SMS consent box below is never pre-checked; each contact must check it personally to enroll.
